Flu Vaccinations Offered At Madison Schools

The Jefferson County Health Department will be hosting an on-site flu vaccination clinic at Madison Consolidated Schools the week of November 15th-19th. Deadlines begin November 8th and paper copies are being sent home as well. Please take a moment to check your student's backpack, binder, folder, etc.

The information can be found through this link. The consent/permission form is located in a link within the letter itself. Please see the information specific to your student's school.

Should you have any questions, please contact your school nurse. Their email addresses are included in the letter.
